Hear Me Out Speech Pathology is committed to protecting your privacy and handling your personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
To provide speech pathology services, we collect personal information about you, your child and, where relevant, your family. This information may include contact details, medical and developmental history, educational information, assessment results, therapy notes, reports, funding information, billing details and correspondence with other professionals involved in your child's care.
Information is collected directly from you through conversations, intake forms, assessments and therapy sessions. With your consent, information may also be obtained from and shared with relevant health professionals, educators, support coordinators, funding bodies and other service providers involved in your child's care where this is necessary to provide safe and effective services.
Client information is stored securely using our clinical management system, Splose, and may occasionally be held in hard copy where required. We take reasonable steps to protect all personal information from unauthorised access, misuse, loss or disclosure.
Approved third-party auditors (for example, NDIS or Medicare auditors) may access client records where required for compliance purposes. These parties are bound by strict confidentiality obligations.
You have the right to request access to the personal information we hold about you or your child and to request correction of any inaccurate or incomplete information, subject to applicable legislation.
Where required by law, Hear Me Out Speech Pathology may disclose personal information without consent, including where there are concerns regarding the safety or wellbeing of a child or another person.
With your consent, audio and/or video recordings may be used during assessments or therapy sessions for clinical purposes, including assessment, therapy planning, supervision and monitoring progress. These recordings are stored securely and are not shared outside Hear Me Out Speech Pathology without your written consent unless required by law.
Client records are retained for the period required by Australian legislation and professional standards before being securely destroyed or permanently de-identified.
